If you’re considering a 360 feedback program, survey assessment and/or coaching programs for the first time, take the time to plan carefully. Have you tried a program before without the results you wanted? You may have overlooked some important factors. This checklist may help you with planning and implementation.

1. There is a visible, sponsoring leader that is committed to the process   
2. Program objectives have been clearly articulated and agreed upon  
3. The organization is “ready” for this process, e.g. culture  
4. Questions / demographic categories have been developed or agreed upon  
5. The people who will see the results have been determined  
6. The start-to-finish process has been defined with timing & dates  
7. The software tool has been selected and tested

8. Administrator trained and in place  
9. Software application loaded with participants, email addresses, questions, logos, welcome message, etc.  
10. Communications written and distributed  
11. Orientation material written and meetings held as appropriate

12. Invitation emails sent with website and passwords  
13. Initial monitoring and correction of any email errors  
14. Monitor real-time participation and send reminder emails as scheduled  
15. Close-off or extend deadline  
16. Reports produced and distributed  
17. Follow-on programs conducted, e.g. lprCoach
